Member Cancellation/Freeze Form

  1. Cancellation Policy: "Month-to-month" membership cancellation notices must be submitted in writing (digital submission) no later than 24 hours prior to your scheduled electronic payment deduction date. Electronic payment deductions will proceed as scheduled until a written cancellation notice is received. "Package Plans" membership cancellation notices must be submitted in writing (digital submission) no later than 24 hours prior to your scheduled electronic payment deduction date prior to renewal date. All "Package Plans" must be paid in full before cancellation.  

  2. Refund Policy: Refunds are not issued for membership payments. Members are responsible for fulfilling the payment obligations in this agreement until a written membership cancellation notice has been submitted. Cancellation notices submitted after a monthly electronic payment deduction has occurred are not eligible for refunds.

  3. Medical Conditions & Injuries: Any medical conditions, illnesses, and/or injuries that prevent a client from working out or participating in the program shall be validated by a doctor's note indicating your restrictions and length of absence.

  4. Membership Freeze: A membership freeze is a temporary payment hold that is only permitted for memberships that are interrupted by medical conditions, illnesses and/or injuries with a valid doctor's note. In such instances, a temporary membership freeze will take affect starting the month following the receipt of your medical notice.
